Welby Health + Physicians Medical Center Partnership Overview

PMC has chosen to partner with Welby to provide wrap around care for patients who need extra support between office visits. PMC does not have adequate staff time to provide all the services that Welby specializes in.

These services could include:

• Frequent phone calls to follow up from a nurse that has access to your records
• Review of vital signs
• Development of a care plan to optimize your health
• Education about your chronic condition and how to manage it
• Education about resources in our area that you can utilize
